Uploaded a Picture but no Button to Insert It

I've been working on a new post and have uploaded several pictures and successfully inserted them into the post. But now with the last picture the upload seems like it succeeds but there is no button to insert it. What am I doing wrong?

Is there a limit to the number of pictures you can insert?
