Ugh. I used to have a morbid fascination with reading entry after entry in the American Alpine Club accident reports database. Two things I learned - tie a knot in the end of your damn rope, and objective hazards always win.

Looks like he was using a pry bar or something to clear loose rock. Not actually climbing, thankfully.
